Basketball Recruiting LLC Basketball Training Camp Waiver & Release

By training with us, you agree to these terms:

1. Assumption of Risks: Participation in the Basketball Recruiting LLC basketball training camp carries inherent risks, including the potential for physical injuries. Some of these injuries can occasionally be serious or even life-threatening. I recognize and accept all risks associated with my child's participation in the event.

2. Release from Liability: I, as the parent or guardian of the participant, hereby release and agree to hold harmless Basketball Recruiting LLC, its owner Donald Stephen Sackmann Jr., and all affiliated trainers, coaches, volunteers, and staff from any claims, liabilities, damages, or rights of action, present or future, whether the result of injuries, negligence or any other cause arising from or related to my child's participation in the camp.

3. Medical Authorization: Should my child require medical attention while attending the camp, I grant permission to Basketball Recruiting LLC staff and their designated agents to provide necessary medical care.

4. Proof of Insurance: I confirm that my child is currently covered by active medical insurance that will remain in force for the duration of the camp.

5. Media Consent: I consent to the taking and use of photographs and/or videos of my child by Basketball Recruiting LLC. I grant Basketball Recruiting LLC permission to use such images for promotional, marketing, and other related purposes, including but not limited to publications, advertisements, brochures, and their website.

6. Membership Billing Policy: If I enroll in a monthly membership, I understand and agree that I will be billed automatically every 28 days until I cancel the membership in accordance with Basketball Recruiting LLC’s cancellation policy.
